This is formular package

The package contains some macros which are helpful to typeset 
forms containing fields which are intended to be filled manually
or using TeX.

Installation:  This archive comes with ready-to-use files.

1. Copy formular.sty to a directory within the LaTeX search path.
2. Read the documentation in formular.dvi
